Well, I didn't know the answer, and couldn't even google it :)

But I DID ask my staff writer with the degree in journalism.  This was his answer:

"Emphasis added throughout" is a phrase that comes up when an author is quoting someone else and wants to strengthen part of a quote that wasn't given additional emphasis in the original. Does that make any sense? Here's an example I just made up:

Original quote by Bob Smith:
"At the start of the year 2000 military spending was at its lowest level in thirty years. By the end of the year, however, spending had tripled."

And here's that quote being used by someone else (Jason Jones) with the emphasis added throughout:

"Our military spending is clearly out of control. According to Bob Smith, 'At the start of the year 2000 military spending was at its lowest level in thirty years. By the end of the year, however, spending had tripled ' (emphasis added throughout)."

If you put part of a quote in italics (or bold, etc) that the original author did not, you're supposed to so say in academic and news writing. The same would be true in Biblical scholarship as well.

Hope this explains it.
